Where can I find the vendor forms or documents uploaded by providers?

Please log into your school profile > left menu > Manage Providers & Programs > Review Vendor Forms

Forms and images uploaded from the providers' profiles can be found here.

 

NOTE:

You will only be able to see the forms uploaded by a provider if your account is authorized by the providers from their end.  Please reference steps 6 & 7 of this help article on how providers can authorize a school coordinator to access their forms.
